Consent to Receive Text Messages

By providing your mobile phone number to Array Medical Imaging, LLC (“AMI”), you consent to receive text messages and other electronic communications related to your care, appointments, and administrative services.

These communications may include:

• Appointment reminders

• Scheduling confirmations

• Appointment updates or changes

• Administrative notifications

• Billing-related communications

• Requests for follow-up contact

• General service-related information

Text messaging consent is not required to receive medical care or services.

Electronic Communication Risks

While AMI uses reasonable safeguards to protect patient information, text messaging and electronic communications may involve inherent privacy and security risks.

Text messages may:

• Be transmitted over unsecured networks

• Be accessed by unauthorized persons if others have access to your phone or device

• Be delayed, intercepted, misdirected, or fail to transmit

• Remain stored on mobile devices or carrier systems

By consenting to text messaging, you acknowledge and accept these potential risks.

Protected Health Information (PHI)

AMI will make reasonable efforts to limit sensitive medical information included in text communications.

However, text messages may include limited information necessary for administrative or scheduling purposes, such as:

• Appointment dates/times

• Office instructions

• Requests to contact the office

AMI will not intentionally send highly sensitive medical details through unsecured text messaging unless otherwise authorized or required.

Patient Responsibilities

Patients are responsible for:

• Providing an accurate mobile phone number

• Updating AMI regarding changes to contact information

• Securing access to their mobile device

• Notifying AMI if they wish to discontinue text communications

Patients understand that anyone with access to their phone or messages may potentially view communications from AMI.

Opt-Out Rights

Patients may opt out of non-essential text communications at any time by:

• Replying STOP when applicable

• Contacting AMI directly

Opting out of text messaging may affect AMI’s ability to provide appointment reminders or administrative updates electronically.

Carrier & Technical Limitations

Message and data rates may apply depending on the patient’s mobile carrier or service plan.

AMI is not responsible for:

• Carrier delays

• Failed message delivery

• Technical malfunctions

• Unauthorized access beyond reasonable safeguards

• Service interruptions related to third-party providers or communication systems

Emergency Communications

Text messaging should not be used for:

• Medical emergencies

• Urgent medical concerns

• Time-sensitive medical conditions

Patients experiencing a medical emergency should call 911 or seek immediate emergency medical care.

Consent & Authorization

By providing your mobile phone number and agreeing to receive text communications, you acknowledge that:

• You have read and understand this SMS/Text Messaging Consent

• You authorize AMI to communicate with you electronically regarding permitted matters

• You understand the potential privacy and security risks associated with electronic communications
