Cancellation, Rescheduling & Deposit Policy

Appointment Deposits

Certain appointments and services provided by Array Medical Imaging, LLC (“AMI”) may require a deposit at the time of scheduling.

Deposits are used to reserve appointment times, allocate staff and equipment resources, and reduce scheduling disruptions.

By scheduling an appointment requiring a deposit, the patient acknowledges and agrees to the terms outlined in this Policy.

Non-Refundable Deposits

All appointment deposits paid to AMI are non-refundable.

Deposits will not be refunded for any reason, including but not limited to:

• Appointment cancellations

• Missed appointments

• Late arrivals

• Failure to appear

• Schedule conflicts

• Transportation issues

• Illness

• Personal emergencies

• Changes of mind

• Insurance issues

• Dissatisfaction unrelated to service performance

• Failure to follow preparation instructions

• Requests to cancel after scheduling

This policy applies regardless of the timing or reason for cancellation.

By submitting payment, the patient confirms understanding and acceptance that deposits are non-refundable under all circumstances permitted by applicable law.

Rescheduling

AMI may, at its sole discretion, allow appointment rescheduling depending on scheduling availability and operational considerations.

Approval of a rescheduled appointment does not convert a deposit into a refundable payment.

Repeated cancellations, rescheduling requests, missed appointments, or non-compliance with office policies may affect future scheduling eligibility.

Late Arrivals & Missed Appointments

Patients arriving late may be:

• Rescheduled

• Subject to shortened appointment times

• Unable to be seen depending on scheduling constraints

Missed appointments (“no-shows”) may result in forfeiture of deposits and/or limitations on future scheduling.

Patient Responsibility

Patients are responsible for:

• Reviewing appointment details before scheduling

• Understanding deposit requirements prior to payment

• Providing accurate contact information

• Arriving prepared and on time

Submission of payment constitutes acknowledgment and acceptance of this Policy.

Refund Exceptions

Refunds will only be issued when required by applicable law or when AMI independently determines that a refund is appropriate under exceptional circumstances at its sole discretion.

Nothing in this Policy obligates AMI to provide refunds outside of legally required situations.

Service Limitations

Appointment deposits reserve scheduled time and operational resources and are not guarantees of:

• Diagnostic findings

• Medical outcomes

• Insurance reimbursement

• Treatment recommendations

Policy Changes

AMI reserves the right to modify or update this Policy at any time without prior notice.

Updated versions will be posted on the website and/or made available at the office.
