AMI may use and disclose your medical information for the following purposes:

Treatment

To provide imaging services and coordinate care. This may include sharing your imaging results with contracted radiologists or other healthcare providers involved in your care.

Payment & Account Administration

For processing self-pay charges, collecting payment at the time of service, maintaining financial records, and managing patient accounts.

Healthcare Operations

For administrative activities such as quality assurance, staff training, business management, legal compliance, and internal review.

As Required by Law

AMI may disclose your information when required by federal or California law.

Appointment & Administrative Communications

AMI may contact you regarding:

• Appointment scheduling

• Report availability

• Administrative matters

We will not leave detailed medical information on voicemail without your permission.

Your Rights

You have the right to (Requests must be submitted in writing.):

• Request a copy of your medical records

• Request corrections to your records

• Request restrictions on certain disclosures

• Request confidential communication methods

• Receive a paper copy of this Notice
