Array Medical Imaging, LLC (“AMI”) is committed to treating all patients with dignity, respect, privacy, and professionalism. Patients receiving services at AMI have the following rights:
Patients have the right to receive considerate, respectful, and professional care without discrimination based on:
Race
Color
National origin
Religion
Sex
Gender identity
Sexual orientation
Disability
Age
Marital status
Source of payment
Any other protected classification under applicable law
Patients have the right to:
Privacy during examinations and discussions
Confidential handling of Protected Health Information (PHI)
Receive a copy of AMI’s Notice of Privacy Practices (NPP)
Request confidential methods of communication when reasonably possible
AMI maintains safeguards designed to protect patient information in accordance with HIPAA and applicable California privacy laws.
Patients have the right to receive information regarding:
The nature and purpose of imaging services
Preparation instructions when applicable
General information about imaging procedures
Financial policies and self-pay fees when applicable
Diagnostic findings and medical recommendations should be discussed with the patient’s treating healthcare provider.
Patients have the right to participate in decisions regarding their healthcare and imaging services to the extent permitted by law and the scope of AMI services.
Patients may refuse imaging services; however, refusal may affect diagnostic evaluation or medical care recommendations from their healthcare provider.
Patients have the right to request access to or copies of their medical records in accordance with applicable law and AMI policies.
Written requests may be required.
Applicable fees permitted by law may apply.
Patients have the right to:
Ask questions regarding services or policies
Voice concerns or complaints without retaliation
Receive information regarding complaint procedures
Filing a complaint will not affect access to care or services.
Patients may also file complaints with the U.S. Department of Health and Human Services regarding privacy concerns.
Patients have the right to receive services in a professional and reasonably safe environment that supports patient dignity, privacy, and safety.
To help ensure safe, respectful, and efficient care, patients are responsible for the following:
Patients are responsible for providing complete and accurate:
Identification information
Medical history when requested
Contact information
Referring provider information
Insurance information when applicable
Patients should promptly notify AMI of any changes.
Patients are responsible for:
Following imaging preparation instructions
Following office policies and safety guidelines
Cooperating during imaging procedures when reasonably possible
Failure to follow instructions may affect imaging quality or appointment completion.
Patients and visitors are expected to:
Treat staff, patients, and visitors respectfully
Avoid disruptive, threatening, abusive, or inappropriate behavior
Follow office rules regarding privacy, safety, and photography restrictions
AMI reserves the right to refuse non-emergent services in cases involving unsafe, threatening, disruptive, or inappropriate conduct.
Refrain from unauthorized photography, video recording, or audio recording within the facility
To help protect patient privacy, staff privacy, confidential information, and the integrity of medical services, unauthorized photography, video recording, or audio recording inside the facility is prohibited unless expressly authorized by AMI management.
AMI reserves the right to request discontinuation of recording activities, restrict access, ask individuals to leave the premises, refuse non-emergent services, or involve law enforcement when necessary to maintain a safe, respectful, and private environment.
Patients are responsible for:
Arriving on time
Providing notice of cancellations when possible
Understanding that repeated missed appointments may affect future scheduling availability
Patients are responsible for:
Payment of applicable self-pay fees
Outstanding balances
Understanding financial policies prior to services when possible
AMI provides ultrasound imaging services and does not provide emergency medical care.
Patients experiencing medical emergencies should call 911 or seek immediate emergency medical attention.
Imaging services do not replace ongoing care from a physician or licensed healthcare provider.
